Great North Care Record

The Great North Care Record is the development of access for health professionals to view your electronic medical records that will over time include a range of healthcare information to help improve the care you receive in the North East. The information shared will be a list of diagnosed conditions so healthcare professionals have a complete record of your care, medication, allergies, test results, referrals, clinic and discharge information, address and telephone number.  The aim is to ensure patients have more joined up care when they receive care from other NHS healthcare providers including hospitals, out of hour’s doctors, NHS 111, etc.

Your records will still be confidential and only looked at by people who are directly involved in your care and your consent will be sought prior to your record being viewed at the point of care.

You can opt out by picking up an opt out form at the surgery or download from here. However, sharing your medical information will make it easier to provide the best care and support for you when you need it most.
